This contract is an agreement between Jae Farkas, M.S., D.D., (referred to hereafter as "the officiant"), and ___________________________ and ____________________________(referred to hereafter as "the couple").

The officiant will provide the following services: free initial consultation, any necessary phone calls, e-mails and contact between the couple and the officiant in order to create a personalized wedding ceremony. The arrival of the officiant at least one-half (½) hour ahead of the scheduled ceremony time at the wedding site in order to be available for moral support, guidance, review or assistance to the couple prior to the wedding.

The officiant will sign the legal marriage license and oversee the witnesses' signatures. The officiant will perform the wedding ceremony. The officiant will provide a personalized marriage certificate and an album containing a script of the couple's ceremony. The officiant will remain for approximately one-half (½) hour after the ceremony to be available to pose for any photographs at the couple's request. The couple agrees to allow the officiant to use images from their wedding in the officiant's marketing materials including but not limited to brochures and websites.

In exchange for the above services the couple will pay the following fees:

Ceremony: $350.00 total

Deposit: $100.00 (non-refundable to hold the date)

Balance: $250.00 (due one week--7 days--prior to the date of the

wedding ceremony by cash, check or PayPal (extra fee

applies).

The following additional fees will be added ONLY if applicable:

Rehearsal: $100.00

Travel Time & Mileage: $1.25 per mile beyond a 50 mile round trip to and

from the wedding site from the officiant's home base of

Santa Clarita, California (calculated using MapQuest)

Premarital Counseling: $65 per session payable at the end of each counseling

session

We accept the above terms and our fee obligations regarding the performance of our wedding ceremony:
